If you have an HSA checkbook, just write out a check for your account balance, made payable to the AHR Administrative Account, and mail it to us with a completed  enrollment form!  (Click here ->Enrollment for the form)

or

If you have another type of account, simply print out the transfer letter (on the "forms" page) and send it to your HSA administrator.  Then mail us copy of that letter, along with a completed enrollment form!
